Planning for Discharge
We understand that being returning home (or establishing a new home) after a stay at The Retreat at Sheppard Pratt may seem daunting. One of our main goals is to work with our residents to design a holistic aftercare plan that will be effective for them in maintaining a healthy lifestyle. We have a team of three highly trained, dedicated social workers who work directly with residents to help them chart their paths for a meaningful future.
During their stay at The Retreat, residents meet individually with a social worker to address all of the components that contribute to a healthy life, and determine how to ensure those components are still focused on after discharge. We work to replicate the detailed, personalized treatment plan developed by our care team that is working for each resident. We help our residents find:
- Outpatient psychiatrists, who we fully brief prior to the resident transitioning to their care
- Outpatient therapists that offer modalities that work for each individual resident
- Complementary treatment, such as acupuncture, yoga, tai chi, or other offerings that have been effective during their stay at The Retreat
We also work alongside our residents to help them plan for everyday life, from helping find volunteer opportunities, to assisting with selecting and signing up for continuing education classes, to helping obtain a driver’s license, to providing guidance throughout the relocation process.
